German hermaphrodites push for human rights

Christiane Voelling is a 52-year-old intersexual who lives in Dusseldorf, Germany and has fought for greater rights for people like herself whose sexual gender is indeterminate.

MAINZ, Germany ? Pink? Or blue? For most parents this is the paramount question when it comes to organizing a baby shower or choosing a color for a newborn's room.

But, what happens if the exact gender of the child cannot be determined? It is estimated that in Germany alone approximately 80,000 people are intersexual, so-called hermaphrodites, who have physical features ? such as chromosomes, hormones, gonads and outer sexual organs ? which cannot be unambiguously attributed to just one gender.?


Christiane Voelling, 52,?is an intersexual.

She is a nurse living in D?sseldorf who was born without defining gender characteristics.

Because German law requires that a newborn's personal data ? including gender specification ? is registered within a week, Christiane was proclaimed a boy at birth and called Thomas after a midwife supposedly mistook her enlarged clitoris for a penis.

In Voelling's case, it was later diagnosed that her indeterminate external genitalia were the result of a rare genetic disorder of the adrenal gland, the so-called congenital adrenal hyperplasia, or CAH.

"My childhood and teenage development was often agonizing because I did not really know what was wrong with me and where I belonged," Voelling said in a recent interview with NBC News.?

Following a push from various human rights groups, Germany?s government commissioned its National Ethics Council in December 2010 to consider the issue and come up with recommendations on how to identify intersexuals so they could live with greater dignity. The council, which in February of this year released its recommendations, even grappled with the question of whether or not a ?third sex? should be introduced.??

Invasive surgery
For Voelling, her gender issues were rarely discussed in her family growing up in a small town in a rural area of western Germany since her parents were convinced that they were raising a male child. Yet, in school, she would often be reminded of her ambivalence, when she played soccer with the "other boys," but also felt very much integrated in the girls clique.?

She was experiencing an inner conflict that is rather common among intersexuals, or so-called differences of sex development (DSD) affected individuals, experts say.

"By the mid 90s, the first intersexual patients started seeking psychological help at our offices and most of them were preoccupied with feelings of shame, humiliation and a burdening tabooization of their problems," said Dr. Sophinette Becker, a sexologist and psychologist from Frankfurt.

For Voelling, her emotional trauma grew significantly larger at age 18, when physical scars were added in an unnecessary operation.

After being admitted to a local hospital for an appendix surgery, doctors diagnosed that their patient had mixed male-female genitals and an atrophied reproductive system.

But, when the young adult landed on the operating table, the surgeon found a full set of female reproductive organs, including an intact womb and ovaries.

Without consent from the patient, the organs were removed.

"I never received a truthful explanation of my condition and after the operation I felt a lot of physical and emotional pain for many years,"? Voelling said.

"Some 95 percent of all intersexuals systematically undergo genital surgery and other interventions without medical informed consent and without clear scientific proof," said Lucie Veith, the head of "Intersexuelle Menschen eV" in Hamburg, a group that represents hermaphrodites in Germany.

Gratification after legal battle
Only a couple of years later, Voelling also started receiving the regular administration of testosterone, or steroid male hormones.

"For 27 years, I was more or less exposed to severe doping," Voelling said.

"At age 47, when I felt more like a woman than a man anyway, I said enough is enough," she added.

Today, intersexual activists are trying to educate the medical community, affected families and the public about the often harsh consequences of genital reconstruction surgery and other severe medical interventions.

"These massive medical interferences plunge the intersexed child into total imbalance and lead to irreversible damages," said Veith, whose organization has nearly 600 members in Germany.

In 2008, Voelling decided to take her case to court and sued the doctor that had removed her female reproduction organs over unlawful intervention.

In its verdict, the court ordered the surgeon to pay 100,000 euro, (approximately $133,000)? in compensation for performing an operation converting a hermaphrodite into a man without consent.

"I felt very relieved and it was really more of a moral reparation than anything else, but it unfortunately did not have consequences for the legal rights of intersexuals," said Voelling.? She officially changed her gender from male to female, as well as her name from Thomas to Christiane, in a long bureaucratic process that same year.

Preparing legal framework
While experts say that Voelling's case is legally unique and will not set a precedent, the topic nevertheless started to receive more public attention after she wrote a book called "I was man and woman ? My Life as an Intersexual.?

Despite some disagreements with the recommendations of Germany?s National Ethics Council released last month, Voelling and other intersexuals hope that the council?s recommendations will help give their status a legal framework in the future.

"In our recommendation to the German government earlier this year, the main message was that intersexuals are different from other human beings, but they need to be respected and belong in the center of our society," said psychologist Dr. Michael Wunder, a member of the German National Ethics Council.

Because irreversible medical interventions of gender assignment in people with ambiguous genitalia are typically conducted during early childhood years, the German Ethics Council determined that these operations present an infringement of the right to physical integrity, thus a violation of basic human rights.

The Ethics Board also said that "a non-justifiable encroachment on the personal rights and the right to equal treatment is present when people who cannot be assigned as ?female? or ?male? because of their physical condition are legally compelled to assign themselves to either category in the civil registry."

Following Australia's example, the German Ethics Council recommended that in addition to the registration of? "female" or "male," the German government should introduce the category "other" or should allow a ?no entry,? until the affected person have made a personal decision themselves.

Last September, Australia? introduced new guidelines, which allow its citizens to change the sex details on their passport to female (F), male (M) or indeterminate (X).

"This amendment makes life easier and significantly reduces the administrative burden for sex and gender diverse people who want a passport that reflects their gender and physical appearance," said Australian Foreign Minister Kevin Rudd.

In Germany, rights activists, representatives from the Ethics Council and intersexuals now hope that German lawmakers will soon implement regulations, which will help to protect the rights of hermaphrodites and remove discrimination on the grounds of gender identity and sexual orientation.

Science-Based Medicine ? A Universal Anti-Cancer Drug?

We frequently deal with fraud and quackery on this blog, because part of our mission is to inform the public about such things, and also they are great examples for explaining the difference between legitimate and dubious medical claims. It is always our goal not just to give a pronouncement about this or that therapy, but to work through the logic and evidence so that or readers will learn how to analyze claims for themselves, or at least know when to be skeptical.

One skepticism-inducing red flag is any treatment that claims to treat a wide range of ailments, especially if those ailments are known to have difference causes and pathophysiologies. Even claiming that one treatment might be effective against all cancer is dubious, because cancer is not one disease, but a category of disease. We are fond of pointing out that there are many types and stages of cancer, and each one requires individualized treatments. As an aside, it is ironic that CAM proponents often simultaneously tout how individualized their treatment approach is, but then claim that one product or treatment can cure all cancer. Meanwhile they criticize the alleged cookie-cutter approach of mainstream medicine, which is actually producing a more and more individualized (and evidence-based) approach to such things as cancer.

In any case ? my immediate response to any article or website claiming to treat most or all cancer is to be highly skeptical, but I reserve final judgment until after I read through the details. What kinds of evidence are being presented to support the claims, and what are the alleged mechanisms of action? Are those making the claims being cautious like a scientist should, or are they being promotional like a used-car salesman?

A recent study claiming a potential treatment for many types of cancer has been making the rounds. The title of the article being circulated is, One Drug to Shrink All Tumors. What made me take immediate interest in this article was that it was not on a dubious website, sensational tabloid, or even mainstream news outlet, but on the news section of the American Academy for the Advancement of Science (AAAS) website. This is a report of serious medical research. The title, I suspect, is perhaps a bit more sensational than it otherwise would have been because of a geeky nod to the ?one ring to bind them all? Lord of the Rings quote. Regardless of the source and the headline ? what is the science here?

What scientists have discovered over the last decade is that some blood cancer cells (leukemia and lymphoma) express on their surface a protein called CD47. This protein acts as a signal to the immune system and prevents it from attacking the cancer cells. The same CD47 protein is expressed on normal blood cells and keeps them safe from the immune system as they circulate around the body. It makes sense that those neoplastic cells that by chance express CD47 are the ones that will survive and develop into a cancer. Those that do not express the protein are probably weeded out by the immune system. Cancer cells are therefore the Darwinian survivors, although not beneficial for the host.

The AAAS is now reporting on a new finding of Dr. Irving Weissman, who made the original CD47 discovery:

?What we?ve shown is that CD47 isn?t just important on leukemias and lymphomas,? says Weissman. ?It?s on every single human primary tumor that we tested.? Moreover, Weissman?s lab found that cancer cells always had higher levels of CD47 than did healthy cells. How much CD47 a tumor made could predict the survival odds of a patient.

Not exactly all tumors, but every one tested so far. Still, this is an amazing discovery. It seems that expression of CD47 is a universal or near universal property of all cancer cells, perhaps required for their ability to evade the immune system long enough to become clinically detectable. Weissman and his team then took the next logical step, they created anti-CD47 antibodies and then tested their activity against tumors. They transplanted ?human breast, ovary, colon, bladder, brain, liver, and prostate tumors? into mice, and then compared untreated mice to those treated with anti-CD47 antibodies. What they found is that those mice that were treated generally experienced shrinkage of their tumor and a marked decrease in the risk of the tumor spreading. This is not the same thing as a cure, but any treatment that shrinks tumor and prevents metastasis is a useful treatment against cancer and is likely to prolong survival, decrease recurrence, and increase the probability of a cure.

Our experience with past treatments that show similar promise is that they turned out not to be outright cures, but those that worked became useful additional treatments that improve outcome in some cancers. If anti-CD47 antibodies pan out as an effective treatment, it is likely that they too will be added to the anti-cancer armamentarium, but will not be cures by themselves.

So far, so good ? we have what sounds like a very plausible mechanism, and reasonable animal data. Further, the scientists express the proper degree of caution and skepticism toward their own research. They point out that anti-CD47 antibodies also allow the immune system to attack healthy blood cells, causing anemia. The decrease in blood counts was short-lived in the mice, but obviously this will be something to watch carefully in human trials. The also report:

?The microenvironment of a real tumor is quite a bit more complicated than the microenvironment of a transplanted tumor,? he notes, ?and it?s possible that a real tumor has additional immune suppressing effects.?

Another important question, Jacks says, is how CD47 antibodies would complement existing treatments. ?In what ways might they work together and in what ways might they be antagonistic?? Using anti-CD47 in addition to chemotherapy, for example, could be counterproductive if the stress from chemotherapy causes normal cells to produce more CD47 than usual.

Exactly ? animal models are great preludes to human studies, but they are not the same as human data. Biological systems are complex, and we cannot predict what factors may affect the response of a native human tumor to this treatment based upon evidence from transplanted tumors. Further, there are other factors that may impact the net effect of this treatment in humans with cancer, such as interactions with chemotherapy or other host responses.

Therefore, there is no substitute for controlled human trials. We cannot and should not base claims of efficacy, or even safety, on a plausible mechanism and encouraging animal data. What we have so far is sufficient justification for phase I clinical trials in humans, and nothing more.

This is a point worth emphasizing, as it is very common for dubious treatments and products to base clinical claims on preliminary basic science or animal data, often backed up by nothing but anecdotes. Together this kind of evidence can make a compelling story to the non-scientist, and that?s what the marketers are counting on. It is helpful to see how real scientists approach the evidence, and the process that is necessary before we can know that a treatment is safe and effective.

Anti-CD47 antibodies are definitely a promising discovery and a treatment to watch. I hope the research pans out and we can add this to our available treatments for cancer. It is possible, even probable, that (if the treatment proves safe and effective) in the future we may test cancers for the presence of CD47, and then then give the treatment only to those who express this protein ? further individualizing cancer treatment. Human studies are likely to take 5-10 years, however, and that?s if everything goes well.

Don?t worry all you people-centered data-haters (yes, I put myself in this category) , you?ll get it too.? I actually had to take a little extra time with these sections because numbers don?t come naturally for me and I really wanted to ?get it?.

Can?t afford to pay for Harvard Business School or don?t have the time to take away from your business?? Don?t worry.? Get this book instead.? It?s the course and all it?s lessons in a short book.? It?s not exactly easy or fun to read.? I like to say that this isn?t a ?snuggly? book.? It?s a serious book that you will want to consume a chapter at a time, take notes and maybe collect some additional information.

Strategy is something not to be taken lightly and that is the overarching lesson that Montogomery communicates between the covers of this brilliant book.

Like most fighters who have achieved a similar level of greatness, 14-1 UFC champion Junior dos Santos isn?t out to prove he?s second best. After a storied run through the division the heavy-handed Brazilian has established himself as the sport?s top heavyweight with one possible exception ? Alistair Overeem. Fortunately for ?Cigano?, Overeem also happens to be his next opponent, giving Dos Santos a unique opportunity to separate himself from the pack to enter territory currently reserved for the likes of Anderson Silva and Georges St. Pierre.

Dos Santos understands how important beating Overeem would be for his career. However, that doesn?t mean he?ll approach him any differently than he would any other opponent including a willingness to exchange despite the Dutchman?s powerful, polished stand-up.

?I think it?s the ultimate test for my striking,? said Dos Santos in an interview with Sherdog. ?He was a K-1 champion, which is the greatest striking event in the world. He?s a very strong guy, good, but I trust in myself a lot and I try for the knockout in all of my fights. Regardless of him being a striker or not, I?m going for the knockout.?

Dortmund beats Fuerth with last shot of extra time

FRANKFURT, Germany (AP) -Substitute Ilkay Guendogan scored with the last shot of extra time for Borussia Dortmund to win 1-0 at second division Greuther Fuerth on Tuesday and reach the final of the German Cup.

Guendogan's shot from the edge of the penalty box hit the post and bounced off the back of goalkeeper Jasmin Fejzic into the net. Fejzic had come on just a couple of minutes before, with Greuther Fuerth believing the match was going into a penalty shootout.

Fejzic is renowned for his penalty-saving skills and can also score from the spot.

In the second semifinal, titleholder Bayern Munich visits Borussia Moenchengladbach on Wednesday.

Dortmund, the Bundesliga champion and front-runner, stayed on course for a domestic double and its third German Cup title.

"It was incredibly difficult," Dortmund's coach Juergen Klopp said. "We had to work very hard, they are a very good and very dangerous team. It was exactly what we expected."

After several near misses at promotion, Greuther Fuerth leads the second division and looks set to move up into the top tier.

Borussia dominated early possession and created a series of chances that went to waste.

Although Greuther Fuerth mostly defended well, Shinji Kagawa had an open header from six meters (yards) out sail high. Kagawa and Robert Lewandowski both failed to take advantage of a good cross in the 36th, and Kevin Grosskreutz had a header saved by Greuther Fuerth's goalkeeper Max Gruen.

(AP) ? Kansas' Thomas Robinson knew there could be games like this in the NCAA tournament.

Sometimes it's just a matter of surviving.

The No. 2-seeded Jayhawks did, taking their first lead late and holding on to beat No. 10 seed Purdue 63-60 Sunday night and advance to the regional semifinals for the fifth time in six years.

"Something like that is stuff you have to face to win a championship," Robinson said. "Coach said there is going to be that ugly game. That was as ugly as I've seen."

Elijah Johnson scored 18 points, including the go-ahead basket in the final minute, to help Kansas overcome a monster shooting performance by Purdue's Robbie Hummel.

In the West Regional game, Florida defeated surprising Norfolk State 84-50 after blowing open the game with an early 25-0 run.

Kansas (29-6) heads to St. Louis to play North Carolina State in the Midwest Regional and Florida (25-10) travels to Phoenix to play Marquette in West Regional.

The Jayhawks' Robinson fought through double-teams all night for 11 points and 13 rebounds against Purdue, and they got enough production from everyone else to erase a 10-point second-half deficit.

Robinson, the Jayhawks' national player-of-the-year candidate, was just 2 for 12 and Tyshawn Taylor was 4 of 11.

"If Elijah didn't turn around," Robinson said, "we would have lost that game tonight."

Purdue was clinging to a 60-59 lead and had the ball and under a minute remaining when Lewis Jackson, the shot clock winding down, lost control at the top of the key. Johnson picked it up and went the other way for the go-ahead lay-in with 23.3 seconds left.

Hummel missed an open 3-pointer at the other end and Taylor scored a transition dunk for the Jayhawks with 2.5 seconds left, giving the roughly 15,000 fans who had made the three-hour drive from the Kansas campus reason to let out a roar for one of the first times all night.

After a timeout, Purdue sharpshooter Ryne Smith managed to get off a decent look at a long, potential tying 3-pointer. It hit off the backboard, clanked off the rim and fell away.

"It stinks," Purdue coach Matt Painter said. "It stinks to lose."

Hummel finished with 26 points and nine rebounds for the Boilermakers (22-13), who were trying to reach the round of 16 for the third time in four years. D.J. Byrd and Terone Johnson had 10 points each for Purdue.

The Jayhawks' biggest lead all night was their final one. They overcame a rough night by Robinson by getting 10 points from Taylor and 10 more from Travis Releford.

"Coach said that for us to win this game, we have to make them play bad in the first half. We didn't," Taylor said. "We made eight turnovers, they missed 10 shots in the first half and got six offensive rebounds. So we weren't doing anything to stop them. We kept grinding. When we needed the stops, we knuckled down and got them."

Purdue couldn't have gotten off to a much better start.

Neither could Hummel.

The senior forward hit his first four shots, three of them from beyond the arc, and followed up his first miss with another basket with 11:46 to go in the first half that made it 19-8.

He proved too quick for Robinson to guard and too strong for Kevin Young as the Jayhawks kept searching for anybody who could put a body on him ? they even tried seldom-used Justin Wesley.

The miserable start by Kansas was enough for Self to scream at his team during one defensive trip down floor, "You told me you were ready!"

Hardly seemed to be the case.

Kansas opened the game by missing 15 of its first 17 shots and all seven of its 3-point tries, compounding lousy offense by getting into foul trouble. Taylor, Young and Releford all sat stretches in the first half after picking up two early fouls.

The Jayhawks finally trimmed the lead to 31-30 with under 3 minutes left in the first half, but Lewis Jackson got inside for a basket, and Hummel managed to swish a closely guarded 3 from about 30 feet as the shot clock wound down to make it 36-30 at the break.

Hummel had 22 points on 7-of-8 shooting in the first half, while the Jayhawks' trio of stars ? Robinson, Taylor and Johnson ? managed 12 points on a combined 4-for-18.

"I wanted to come out and be aggressive, especially the first half. It seemed like everything I was taking was going in," Hummel said. "It was a crazy feeling you have as a player."

Earlier in the day, Florida put an end to the feel-good story that was Norfolk State.

Any hope Norfolk State had of matching the upset it pulled against Missouri on Friday vanished as the Gators went on a 25-0 run in the first half.

The Gators had a business-as-usual air about them after dispatching the Spartans (26-10). After all, Florida is 17-2 in NCAA tournament games since 2005-06. Combined with their 71-45 win over Virginia on Friday, the Gators left Omaha with two wins by a total margin of 60 points.

Not too shabby for a No. 7 seed.

"There are certain guys who want this kind of stage," Florida coach Billy Donovan said. "In this tournament you have to have players who play fearlessly on both ends of the floor and are not worried about making mistakes and are competitive and are driven to want to be great."

CNY Arts Center's Annual Arts Fest Planned | Oswego County Today

FULTON, NY ? The CNY Arts Center?s second annual Arts Fest, to be held June 9 at Fulton?s War Memorial Ice Rink, is expected to be even bigger and better than last year?s event.

With more space to work with at the festival?s new location, The Arts Center is revamping the entire event and allowing room for even more vendors.

The 2011 Arts Fest was held by the river in downtown Fulton, which limited vendor space, and made it difficult to consolidate the art exhibits with the food and entertainment.

The new and improved Arts Fest will have music, food, and entertainment all on the same grounds, starting at 10 a.m. and lasting until 4 p.m.

Musicians and entertainers will be performing throughout the day and the Arts Center will also be hosting a talent show. The talent show will be categorized by children, youth, and adults, and assessed by a panel of performing arts judges.

The size of the arena and space outside of the War Memorial will permit the Arts Fest?s committee to invite more artists than last year.

The CNY Arts Center?s Director, Nancy Fox, emphasizes that artists of all ages and artistic styles are encouraged to register. ?More than a dozen artists attended our first Arts Fest, and we want to multiply that number ten times,? said Nancy Fox, organizer.

Artists who wish to register for the event will be given a space to display their work for a nominal fee. The fee will vary slightly based on an indoor or outdoor exhibit area, and priority for placement will be on a first-to-pay, first-served basis.

The Arts Center is looking for any artists who have handmade original art to display and sell. ?Last year?s event featured everything from jewelry, to botanical prints, to beach glass art, to wood carvings, to children?s art projects,? said Fox. ?We want this annual event to grow and become a much anticipated event spotlighting the finest of Oswego County artists.?

Aside from the festival?s art and entertainment, the Arts Center is recruiting an assortment of food choices.

The food vendors to attend will include different restaurants from the Central New York region, including the widely-favored Frickin? Chicken Barbecue.
